My promise is to deliver comprehensive, evidence-based education for all that facilitates comfort, dignity, and emotional security for individuals facing life-limiting illnesses.
I meet individuals and families where they are at. I can accommodate various age groups, community members, or students whether to start a Goals of Care conversation, explanations of various legal documents such as POLST forms and Advance Directives, discuss options for the “what ifs” or to gain a better understanding of diseases processes.
Goals of Care Conversations. What types of medical interventions are desired and when? Who will be the designated spokesperson if you are unable to communicate?
Facilitating Family Communication such as speaking first with each or a few family members about their concerns then gradually adding in more people to discuss a plan of care for a loved one.
